Step 1: Access Forms
1. Click on the Main Menu icon.
2. Select Curriculum.
3. Select Forms.
Step 2: View Form
4. Locate the form that you would like to view and click anywhere on its row to reveal its options. Select View.
View Form Questions
On the View Form screen, you will immediately see the questions appear. If the form has been completed by users, you will see the response choice counts totaled for each question.
View Survey Batches
If you are working with the Survey form type, you will see a section for Survey Batches to the left side of the View Form screen. If the survey has been launched multiple times, you can select to view results for specific batches by selecting the batch date in the Survey Batches section.
NOTE: Batches only apply to forms that are launched via the Actions drop-down menu on the main Forms screen. Surveys are the only form type that require launching in order to make them available to users. All other form types (assessment/portfolio forms, demographic surveys, and course evaluations) are made available to users by selecting the Visible form setting option on the Edit Form screen.
View Form Respondents
The Respondents tab allows you to view all users whom have received the form. You can also see whether a user has completed the form or not and when it was completed. By clicking on the name of a respondent in the User column, you can view the individual form responses.
View Form Information
The Info tab allows you to view which Groups/Departments the form is associated with and which Privilege Levels the form applies to.
